11/12/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C met with to discuss from mother which indicates client does not know how to communicate with his siblings and peers on a daily basis within the home and school setting. Mother reports these behaviors are lasting 2-3 times weekly last for about 10 minutes. 2. Behavior: During the second portion of the session, client needed assistance with being able to verbalize his feelings of frustration as he was getting upset with his mother while she was discussing with QMHP her issues with him when it comes to managing his anger at which he will throw a temper taturm with his mother and QMHP-C while participating in the session which lasted for about 3 minutes until he was able to understand how to implement coping skills at least 2-4 times weekly. 1.Intervention: QMHP-C assessed and monitored Clientâs mood and behavior. QMHP-C probed in order to process and explore Clientâs lack of social skills. QMHP-C educated and reviewed effective communication skills with Client to utilize with family members and others (socially). QMHP-C educated and reviewed appropriate coping strategies with Client to utilize within social settings to prevent Client from withdrawing and distancing himself. QMHP-C modeled appropriate skills and strategies through role play and role reversal. 2. Intervention: QMHP-C encouraged Client to increase his social interactions with others. QMHP-C discussed the importance of positive relationships with his family and peers. QMHP-C encouraged Client to articulate and express his feelings without exhibiting inappropriate and disrespectful behaviors. QMHP-C prompted Client to apologize to Ms. Bates and family for his behavior today. 1.Response: In todaysâs session Client reported that he has been doing what he is supposed to be doing but it is his siblings that he getting him in trouble causing him to get upset and for him to leave the home and go to the playground when he wants to. Client stated that he will not utilize aggression towards his mother nor family members if they just stay out the way. 2.Response: Client reports that he is going to do what he supposed to do when it comes to listening to his mother and his grandmother. He reports that he is not trying to get in trouble with is mother because she will take his phone away from him and that is something that he does not want to happen.
